Perez Physical Therapy does not participate with any insurance networks therefore all services will be processed out-of-network.

 

Payment is expected, in full, at the time of service. As a courtesy, we are willing to submit claims electronically on your behalf, if permitted by your carrier, so that they can process your reimbursement. Most, if not all, correspondence will be sent directly to you by your insurance carrier including reimbursement payments.

 

We will provide you and/or your insurance carrier with all requested documentation to process your claims for reimbursement. We will attempt to acquire any pre-authorizations if required and informed of such requirements.
